Visiting Hospice Nurse

As the team's supervisory nurse, the RN Case Manager manages symptom and pain control, administers necessary medication, provides education and guidance to other nursing staff, and acts as a direct liaison between the patient and physician.

Hospice Medical Director

Certified Home Health Aides (CHHA)

Assists with daily activities, including personal hygiene, grooming, dressing, bathing, and household tasks.

Volunteers

Screened and trained volunteers offer companionship and support to patients and their loved ones, with some volunteers providing respite relief to caregivers.

Bereavement Coordinator

The Bereavement Coordinator provides compassionate grief counseling to those affected by a life-limiting illness or the loss of a loved one.

Medical Social Workers (MSW)

The MSW assesses and champions for the unique needs of the patient and their loved ones. They offer financial advice and support through community resources and may assist in arranging final plans with the family.

Music Therapy

Our dedicated music therapists work closely with individuals and their families to tailor sessions that honor personal preferences and memories, enhancing quality of life and promoting a sense of peace and well-being during this profound transition.

Spiritual Counselors

Provides comforting spiritual assistance and helps patients connect with their own faith community.
