Hospice care involves an inter disciplinary team oriented approach to expert medical care, pain management, emotional and spiritual support designed meet the patients unique and specific needs. Support is also provided to the patients family as well. At cornerstone of hospice and palliative care is the credence that each person has the right to die with dignity and their families also receive the necessary support they require. Hospice care is provided in patients home, freestanding hospice centers, hospitals, nursing homes and other long-term care facilities. Hospice services are available to patients of any age, religion, race, disability or illness. Hospice care is covered under Medicare, Medicare-Cal, most private insurance plans, HMOs, and other managed care organizations.
